File your Colorado Pharmacy Affidavit Today

Today, January 31, 2018, is the last day to sign and return the non-resident pharmacy affidavit to the Colorado Board of Pharmacy, which is available here. The required affidavit confirms the nonresident pharmacy is aware of the Colorado law regarding the distribution of medications into Colorado, and will not engage in the distribution of compounded products to Colorado veterinarians without meeting the detailed requirements. Failure to submit the signed affidavit may result in a complaint against your nonresident pharmacy registration issued by the State Board of Pharmacy.
